What Is ICSI Treatment?

ICSI Treatment is short for intracytoplasmic sperm injection – an infertility treatment that is needed when there is a problem with the sperm. During ICSI treatment, a powerful microscope and a fine glass needle are used to select the best sperm and gently insert it in the centre of the egg.In normal IVF, many sperm are placed close to the egg, and they have to swim to enter the egg. With ICSI, even if there are very few sperm, the sperm move very slowly, or many of them look different or abnormal, couples can still have their own real (biological) baby. This is feasible because of the direct injection.

At Kiran Infertility Center Delhi, ICSI infertility treatment is carried out by highly qualified and experienced embryologists in a cutting-edge lab using high-powered microscopes and micromanipulation tools to aid in the delicacy of this procedure. Every step of the ICSI process is observed with strict safety procedures in order to ensure a safe, precise procedure and uncompromised embryonic health.

Who Should Receive ICSI Infertility Treatment?

Not every couple going through IVF needs ICSI infertility treatment. It's mostly utilized when standard IVF is less likely to operate properly. Doctors at KIC Delhi carefully look at the health of each couple before deciding if ICSI infertility therapy is the right choice. When a male has very few sperm in his semen and regular IVF may not work well, ICSI infertility treatment is frequently recommended for severe oligospermia.

  • Azoospermia is the medical condition of a man whose semen contains no sperm.

  • TESA, TESE, micro-TESE, and PESA are four techniques for collecting sperm directly from testes or epididymis.

  • Asthenospermia: male whose sperm move very slowly and are unable to reach or penetrate the egg on their own.

  • Severe cases of teratospermia: when a high number of sperm are abnormally shaped, making it hard for them to achieve normal fertilization.

  • Couples who had no or little fertilization in previous IVF cycles, despite what appeared to be normal sperm.

  • Couples using frozen sperm, which may lose some of its strength after being frozen and thawed.

  • Couples who are performing Preimplantation Genetic Testing (PGT) so that additional sperm don't mess up or contaminate the genetic sample.

  • Men who have had a vasectomy and are using sperm that was surgically removed to get pregnant.

  • Cases of unexplained infertility where previous IVF treatments did not result in fertilization or did so poorly, with no evident reason.

ICSI Procedure: Step by Step at KIC Delhi

Step-by-step guide to the ICSI Procedure at KIC Delhi You can be calm and ready if you know the ICSI method in straightforward steps. The ICSI procedure is part of the IVF cycle at Kiran Infertility Center Delhi. It is done in a smooth, organized, and patient-friendly method.

Step 1: Check your full fertility.

To start, both couples get a full check-up. The woman has hormone blood tests and an ultrasound of her ovaries and uterus, and the doctor looks at her past pregnancies and health problems. The guy offers a semen sample for analyzing the number, movement, form, and sometimes DNA fragmentation of the sperm. The data helps the team make sure that ICSI treatment is right for him and figure out how to collect the sperm.

Step 2: Ovarian stimulation (8–14 days)

She gets fertility shots for about 8 to 14 days so that her ovaries can create more than one mature egg. During this time, doctors often take blood tests and ultrasounds to see how the eggs are growing and change the dose so that enough eggs grow without raising the risk of ovarian hyperstimulation syndrome (OHSS).

Step 3: Getting the eggs (20–30 minutes)

A "trigger" injection is given to mature the eggs fully when the follicles are the right size. After roughly 34 to 36 hours, a little surgery is done to collect the eggs. This task is done with an ultrasound-guided needle and minimal sedation. Most women can go home the same day with just mild discomfort after this egg retrieval, which normally takes 20 to 30 minutes.

Step 4: Get the sperm or pick it up by surgery.

If your semen sample has zero sperm, your Urologist first determines which type of azoospermia (zero sperm) you have. He then collects sperm directly from your testes using available methods such as TESA, TESE, micro-TESE or the PESA method to extract sperm from the minute tube behind the testes, called the epididymis.

Step 5: Washing and screening the sperm sample in the andrology lab

In the lab, the embryologist screens the sperm sample under special microscopes. He then picks the healthiest sperm for shape and motility to use for the injection. In some cases, specialized procedures like PICSI and IMSI (super magnification) are used to help filter out even further sperm samples to improve the chances of success.

Step 6: ICSI microinjection

The chosen sperm is immobilized and aspirated by a very fine glass injection needle. By holding the mature egg with a specialized pipette, the embryologist injects a single sperm directly into the egg’s cytoplasm. These micromanipulation procedures are controlled and performed under a magnification of 200x-400x. This is done for each mature egg retrieved.

Step 7: Fertilization and embryo growth (3–5 days)

The eggs are checked for fertilization 16 to 18 hours after ICSI and if fertilized, now called embryos, they are incubated and monitored for 3-5 days.. Most programs are now equipped with time-lapse systems that allow tracking of the embryos’ development without disturbing them, so the highest-quality embryos can be selected.

Step 8: Embryo transfer

The best embryo is chosen to put in the woman’s womb. Putting the embryo inside is not a difficult thing to do, and it doesn’t hurt. It is performed using a thin and soft tube, and anesthesia is not required. Progesterone and other medications are prescribed to the female partner to prepare the lining of the uterus for implantation and to support it. The other good quality embryos are frozen (vitrified) for use in the future.

Step 9: Take a pregnancy test after about 14 days

Around 14 days after embryo transfer, a blood test is performed, an hCG test, to find out whether you are pregnant.

The KIC team supports the couple in anticipation of the pregnancy test and the stressful period of uncertainty by providing them with emotional support and medical advice.
